Samagra ID, which is notional is also mandatory for a domicile and identity check to be performed.

Income Certificate: This verifies the applicant's income, for which eligibility is assessed. Aadhaar Card : Aadhaar is the pioneer identity document and mandatory for availing majority of government schemes. Ration Card (Madhya Pradesh): This further establishes domicile in the state and entitlement to subsidized food grains. Proof of Residence (Madhya Pradesh): That can be electricity or water supply bills, and property tax receipt. Mobile No.: It has to be a valid mobile number, which will receive SMS regarding status of application and payments. Bank Account Details: The assistance is in the form of cash and is transferred to the beneficiaries account, hence, their active bank account details are required. Passport-sized Photograph: For the applicant's recent photograph, there is a need for a passport-sized photo.
